Your Option

Your healthcare team members who use Missouri Health Connection (MHC) may share your health records with each other for the purpose of treating you, coordinating your care, improving quality or managing the payment for your healthcare.

You have control over health records.  You decide if you want to share them with your health care team members through MHC.  If you decide not to be part of MHC, your doctors will not be able to see your complete health history, even in an emergency situation.

If you would like to allow your health care team to have access and share your health records, please follow the instructions on the OPT IN Form

OR

If you previously chose not to share your health records and you have changed your mind and would now like to allow your health care team to have access and share your health records, please following the instructions here.
